We request that all families purchase a Field Trip T-shirt for their child for $5.00. Students will be expected to wear their shirt on all field trips (unless otherwise indicated by their classroom teacher due to specific dress requirements of a trip). If you are new to PS 39 or your child needs a replacement shirt, please complete this Field Trip Order form and return it to school with $5.00 by Monday, October 6.
Please note: if you already ordered a t-shirt through the school supply program, you do not need to submit this form!
